Based on an original idea by the Neurological Alliance the Society has adapted and
developed a form that you can download and print which provides an outline of what
people need to know about your condition (or that of your loved one) following Encephalitis.
It covers all your basic details and gives you space to describe your needs and
preferences in relation to Eating and Drinking, Cognition, Pain and Sensory changes,
Mobility, Personal Care, Useful Contacts and even a bit about Encephalitis.
